Enrollment for all students new to the district can be accessed online. When completing the online registration process, you will need to provide information regarding your student such as family relationships, emergency contacts, and medical notes. You will need to upload several documents, including the following:
- Birth Certificate. If a birth certificate is unavailable, please contact the school for alternate official documents that meet Kentucky public school student enrollment requirements.
- Proof of a preventative health care examination conducted within one year prior to the initial entry.
- Commonwealth of Kentucky Certificate of Immunization Status
- Proof of an eye examination by an optometrist or ophthalmologist, documented in the Kentucky School Eye Exam Form
- Proof of a dental screening or examination by a dentist, dental hygienist, physician, registered nurse, advanced registered nurse practitioner, or physician assistant, is documented on the Kentucky Dental Screening Form (Parents of kindergarten students have up until January 1 of the kindergarten year to submit the dental screening form).
- Completed Kentucky Preventative Health Care Examination Form.
- If your child will be participating in an athletic activity, you must also submit a KHSAA Middle School Athletic Participation/Physical Examination/Parental and Student Consent and Release. Please note that all six (6) pages must be fully completed, along with required signatures for students, to be eligible to participate.
If your child will be entering kindergarten, you will receive information regarding the date and time to bring them in for Kentucky's Common Kindergarten Entry Screener, the BRIGANCE Early Childhood Kindergarten Screen III. This assessment provides a quick and accurate assessment of a child's development in five areas: Academic/Cognitive, Language, Development, Physical Development, Self-Help and Social-Emotional Development.
Digital Back-to-School Checklist
A digital back-to-school checklist is emailed to all families in July. The digital checklist includes items such as payment of student fees, purchase or rental of middle school Chromebooks, and other items important to your child being ready for the new school year. These forms and payments are due two weeks prior to the start of the new school year.
